ESIR - Pulmonary artery interventions

Treatment of acute pulmonary embolism requires an interdisciplinary approach, with interventional radiology becoming more and more important with the increasing availability of suitable catheter systems for local therapy. The mechanical thrombectomy systems in particular have developed significantly in the recent past and represent a valuable alternative to lysis-based procedures. Chronic thromboembolic pulmonary hypertension develops in 2% to 5% of cases undergoing conservative treatment of pulmonary embolism. Interventional radiology has sharpened its profile for this clinical picture. Pulmonary balloon angioplasty (BPA) represents a supplementary method for surgical desobliteration of the pulmonary arteries. However, a controlled recanalization of the vessels is mandatory in order to avoid major complications for the patient.

In this course, the pathophysiological basics of acute and chronic pulmonary embolism, the diagnosis, and the selection of the appropriate treatment technique are discussed. In addition to practice-oriented case studies, the participants will learn important techniques guided by experienced tutors on suitable simulators.

Lower Limb Endovascular Symposium

ESIR - Thyroid thermal ablation

Thyroid nodules are very common in the general population. Usually, they are asymptomatic, benign at cytology, and can be followed by observation alone in the majority of cases. Infrequently, nodules can grow and cause symptoms, thus requiring treatment. Nowadays, minimally invasive image-guided treatments such as thermal ablation with radiofrequency ablation (RFA), percutaneous laser ablation (LA) or percutaneous ethanol injection (PEI) have been prove to be an effective and safe treatment method, and represent a valid alternative to surgery.

This course will discuss thyroid general anatomy, pathophysiology of benign thyroid nodule formation, and review the most widely adopted minimally invasive treatments, including indications and techniques. Extensive hands-on workshops on various modalities, including laser, radiofrequency, and microwave, will provide delegates with the opportunity to handle current devices and learn about the most advanced technologies in thyroid ablation.

ESIR - Essentials of embolization: Liquid embolization from A to Z

In recent years, liquid embolics have increased in popularity among interventional radiologists, as these agents boast both high embolic efficiency and good clinical results. However, the learning curve is steep, as liquid management and the technical aspects are different and more complex than using conventional coils or particles, and training with these agents should take place in a controlled environment.

Liquid embolics can generally be classified into three main groups: cyanoacrylate (glue), non-adhesive DMSO-based embolics and sclerosants foams. The indications for use, chemical properties, clinical advantages and risks of these can be vastly different from those for sclerosants and cyanoacrylates, and knowing the variances is essential for a well-trained IR.

Training with liquid embolics in real models allows IRs to gain the expertise needed for the management of these products. Due to the similarity between pig and human anatomy, pigs will be used in the in-vivo workshops in order to learn how to deal with the injection of glue and EVOH into livers and kidneys. In the ex-vivo workshops, participants will practice on models to learn about the right way to use sclerosants, cyanoacrylate glue, and EVOH. Additional parallel stations will provide opportunities for participants to learn about the preparation and mixing of glue and EVOH. Organised by Christoph Binkert and lead by a team of expert faculty members, this course will ensure that experienced IRs are up to date on this important and expanding facet of embolization.

ESIR - Reliability in percutaneous tumour ablation

In recent years, minimally invasive cancer treatments have experienced a strong surge, and in some cases, have already replaced open resection as the first-choice treatment. Most notably, thermal ablation has been proven to be an effective method for the treatment of numerous tumour entities in various anatomical locations. In order to achieve maximal reliability, however, interventional oncologists must be properly trained in the application of the various available tools, imaging techniques and image fusion tools.

This ESIR course organised by Prof. Reto Bale will give attendees an overview of the currently available devices and techniques in the field of percutaneous tumour ablation, enabling them to plan and perform procedures and conduct the corresponding follow-up. After a theoretical introduction, trainees will have the chance to test all available devices under the guidance of experienced experts in the hands-on workshops. Participants will also get an impression of the capability of modern 3D guidance devices and image fusion tools. In addition, a recorded case of a stereotactic radiofrequency ablation will be presented and discussed. Afterward, the experts will present their most interesting cases and discuss them with the participants.
